Once you have Adobe Reader installed on your computer, you can click the links below to get the two forms that you will need for your first visit (you can print them directly from Adobe Reader which should open automatically when you click on the links below). The forms print best on a color printer. Once you have printed them, please fill them out and bring them to the first appointment.
Federal law prohibits transfer of confidential medical information without a release signed by the patient or legal guardian. You can download a release form to have your dental records transferred to or from another dentist. Please print the form out and fill in the blanks. Then bring, mail or fax it to the office from which you want to the records to be transferred.
